Do You Need to Set-Up a NEW Account?
The Lakeville Area Public School District uses a system called FeePay. If this is the first time using the system, you will first need to create an account. Click the link for information about how to set up your Account Profile.
We encourage you to add all members of your household and add emergency contacts for your children in anticipation of future registrations within the FeePay system.
**NOTE: IF YOU ARE ELIGLE FOR FREE/REDUCED LUNCH, AND HAVE NOT RE-APPLIED FOR FREE/REDUCED FOR 2019-2020, YOU MUST DO SO BEFORE REGISTERING FOR A SPORT TO RECEIVE REDUCED RATE FOR THIS YEAR.**
Sports Physical Exam Information
How often do I need a Sports Physical? Every 3 years new documentation is required to certify that each student has been medically evaluated and are deemed to be physically fit to participate in school activities.
What if my physical expires mid-season, am I still OK to participate? No, Actually if your Physical Exam will expire during the season, you will not be able to register for the activity on FeePay (the system will lock you out as a warning to get this taken care of).
How do I know when my Sports Physical will expire? You can check the date of your last Physical on File by checking the "Health Tab" of your Parent or Student Portal on Infinite Campus.
Where do I turn in my Sports Physical Form? Please submit your physical to Lisa Heilman in the Activities Office. The form can be scanned and emailed to firstname.lastname@example.org or faxed to 952-469-7945 or dropped off in person.
Where can I get a Sports Physical completed? Your family physician, any minute clinic or a Sports Physical Summer Clinic hosted by Sports Medicine Clinics*
What if I need a Physical very 'Last-Minute'? You don't have to wait to get in with your family clinic if the reservations are backed up. You can walk into any Minute Clinic and the Dr onsite can complete a Sports Physical for you.
Why are the Summer Sports Medicine Physical Clinics a good option? Typically, they are free. They are hosted by Sports Medicine Doctors and Trainers and then you don't have to think about your physical during the school year.
